Most commonly: a damaged internal index (cross-reference table), a bad incremental save, or minor structural corruption — the kind of thing that makes some viewers refuse to open a file that others can still partially read. The first repair pass rebuilds a clean internal structure while keeping your real, selectable text and formatting exactly as they were.
We automatically try a second, more aggressive recovery: rendering every page it can still read as an image and rebuilding a new PDF from those. This recovers the visual appearance even from more seriously damaged files, but the rebuilt pages will be images rather than selectable text — we only fall back to this if the first, better method doesn't work.
You'll get a clear message saying so. Some corruption (e.g. a file cut off mid-transfer, or damaged at the storage level) genuinely can't be recovered by any tool without the original source — we won't pretend otherwise.
